Physical Exam Form
- Students cannot play sports, get work papers or participate on trips with expired physicals. All new students must have a physical on file within 30 days of starting school. Must have ahead of time for preseason.
Asthma Medication Form
- Parent must sign 2nd page! Students may carry their own inhalers.
General Medication Form
- Parent must sign 2nd page! If a student needs to take any other medications in school; Also used for permission to have Tylenol or Motrin in school for pain, fever or headache.
Allergies - Anaphylaxis Medication Form
- Parent must sign 2nd page! Students must bring in their own epipens. We have benadryl in Nurse's Office.

Parents or guardians should call Mrs. Ortiz, the Attendance Officer, before 8:25 AM am each day of a student’s absence (718-584-8328). A student returning after an absence must present to Mrs. Ortiz an Absentee Card signed by either a parent or guardian. This is to be done between 8 and 8:25 am on the morning of the first day the student is back in school. This card is to state explicitly the day(s) and date(s) of and reason for the absence. No student is to go to AM attendance or to class after an absence before reporting to the Attendance Office.

We are pleased to welcome Lessing’s Hospitality Group as our new food service provider beginning in September. Lessing’s has catered past events at the Prep and now joins us as our full-time cafeteria partner. As with previous years, the top question from families relates to food allergies. Rest assured that Lessing’s staff and our new chef manager are well versed in allergy awareness and meal preparation.
