I have read and understand the information contained in the handout entitled "Laboratory Safety Checklist" and I agree to abide by the rules and procedures described there, and explained by my teacher.  I will also abide by any other rules and regulations provided by my Chemistry teacher.  I will do my part to maintain a safe working environment in the Laboratory.
